A Physician’s certification stating your disability first and for most to see if you are eligible to qualify for DISID’s services.

A valid picture ID which could be a Guam issued Driver’s license, Guam Identification card or a passport (not expired).

Psychological Evaluation – Psychologist/Psychiatrist signed documentation stating client’s behavioral/intellectual disability (If applicable)

Once all documentation is obtained, your intake process and be a smooth transition into the program(s) you might qualify for.
